NWI Community Action Corporation to Present Chronic Disease Management Program

Northwest-Indiana-Community-Action-CorporationNorthwest Indiana Community Action Corporation has partnered with the Valparaiso Public Library, 103 Jefferson St., Valparaiso, to offer the Chronic Disease Self-Management Program (CDSMP). The CDSMP is an evidence-based health promotion program developed by Stanford University and the goal of the program is to enable participants to build self-confidence and assume a major role in maintaining their health and managing their own chronic health conditions.

The class is presented in 2 1/2 hour classes, once a week for 6 weeks, starting Weds., Feb. 8th, 1:00-3:30pm at the Valparaiso Library. It is offered as a free, non-credit course, and registration is required by calling 219-794-1829, ext. 2245, or 1-800-826-7871.
