Community Health Needs Survey Open to Lake and Porter County Residents

You have the power to influence local healthcare in fewer than 15 minutes. That’s about how long it takes to complete the Lake and Porter County Community Health Needs Survey.

The results of this survey from Powers Health and Conduent Healthy Communities Institute give Lake and Porter County residents insight into the social and economic drivers of health. First, input is needed from the community.

Powers Health – which includes Community Hospital in Munster, St. Catherine Hospital in East Chicago, St. Mary Medical Center in Hobart and Powers Health Rehabilitation Center in Crown Point – encourages residents of Lake and Porter counties to click the following link and share what they believe are the top issues facing our community: research.net/r/Powers_Health. The deadline for participating in the survey is Nov. 18.

Results of this survey, combined with other community data, will help organizations across the two counties better understand community health concerns, and the feedback will guide efforts for improvement.

This comprehensive assessment takes the conversation beyond disease outcomes and explores the relationship between environment, behavior, illness and length of life.

Survey results will give the community information about what is happening in their neighborhood. Conduent Healthy Communities Institute may invite members of the public to participate in a focus group or listening session.
