would your loved ones do if something happened to you? How would they know your
best interests? A contingency notebook is a binder containing information
that a personal representative needs to know should one become seriously
incapacitated or die. Materials are provided.

Throughout the month of November, VNA of Northwest Indiana will join organizations across the nation hosting community activities in recognition of National Hospice and Palliative Care Month.
Each program is open to the public. Lunch will be provided and seating is limited. Please register using the links provided.
